Job Summary
Advanced Practice Nurses take on the role of primary care provider for chronic care patients. Delivering care directly in patients home or assisted living environments. This position requires detail and organization as well as a superior working knowledge of chronic disease state management as well as pharmaceutical management. This position is competitive with salary, flexible with schedule, works in direct collaboration with Practice Medical Director Malpractice insurance, credentialing, and continuing education are all paid by the practice.
Requirements
- 3 years as RN preferably in Home Health field
- 1 year as APN,
- Full CPT with unencumbered Ohio License
