Putting patient Quality of life front and center is what we do. In bringing that Quality of Life Care Delivery to the next level, AHMP has created the position of Nurse Chronic Care Manager.
This position brings a personal response to our patients with Chronic Care issues when they need it the most. We are excited to announce the addition of Jessica Dimitroff, RN BSN to the AHMP team. Jessica will be over seeing this innovative program and bringing Chronic Care Management to the next level.
