Beginning May 12th, Marc’s pharmacy will contact each registered patient with a time they will be out to administer the vaccine.
Marc’s pharmacy has partnered with At Home Medical Professionals to administer vaccines to our home bound patients residing in Summit county on May 12th. Other counties will be given other dates. Marc’s will reach out to you with a time they will arrive.
You must complete the following:
- The COVID-19 billing and consent form must be completed prior to Marc’s arrival. Please click here for that form.
- Contact Marc’s pharmacy in Fairlawn (they are the coordinating pharmacy) (330) 869-0426 you will tell them you are registering for the At Home Medical in home vaccine clinic.
- Marc’s will call you before May 12th to give you a time for your visit. Please note, there is no flexibility in the time given for your vaccine. There are many restrictions to administering this vaccine, including temperature and time. Your vaccine time is based on the route needed to meet the time and temperature restrictions.
- The Moderna vaccine will be administered. This is a 2 part vaccine. Marc’s will schedule you for a second dose administering about 3-4 weeks after your first vaccine.
